Unfortunately PHP isn't like ASP there isn't a concept of a 'control'. To create a dynamic form builder you will probably be best using Javascript to dynamically add form elements to a form, then upon submission the PHP program will need to iterate through the parameters ($_POST variables) and generate the HTML for the form. You'll need to look into the Document Object Model. If you are unsure about it, it may be best to get somebody else to create this for you.
Why the fffff flamin heck does anyone use PHP?
I have an html script and a php script from a web form builder. I have no idea what to do with the php script!? I tried saving it as .php but it wouldnt open so i tried .html but that didnt work either. I even tried .php.html!!!! Please help I need a step-by-step answer because I only know a little html and no php at all. thanks!

It's spam bud, look at the link.
Seems like an awful lot of work just to spam 1 link that most people will graze over anyway lol.
It's about getting the link to their website for SEO purposes.
