help/guidence on 3d perspective text

Im fairly new to print design, i know a fiar bit about photoshop etc,
but im woundering how to create text like this..

i know that photoshop can make the maltic efects and a basic 3d perspective text but how to this level??

is other softwear used??

3DS Max or Maya could do that, it looks like a 3D application though rather than any of the CS suite so unless you know how to use any 3D apps it could be hard to get it looking exactly like that. You could use a combination of illustrator and photoshop to get some of those effects but it would take ages.
