Missing images

There may not be anything you can do about this and maybe it only happens to me anyway, but i've noticed that sometimes when reading a thread, I get the feeling there's a missing image. It happened to me again today and just to se what would happen, I pressed the quote button. Sure enough in the quoted text was an image URL. I have no idea why sometimes these images don't load, but the real point is, there is no missing graphic icon on the page to let you you know.
I hope I'm making sense.

But to clarify there's nothing wrong with the markup or the image it's just that for some reason on this occaision it didn't load. The point is if an image doesn't load for whatever reason there is no placeholder icon to tell you that there should be an image there. As I say sometimes you know by the context of the post but not always.
